We will contact you via phone, email, or text message regarding your pet’s medical care, including communication from the doctors, expiring vaccinations, and/or upcoming appointments.

My signature below authorizes RPHR to use my Pet(s) photo and/or information related to my experiences. I understand this information may be used in publications, including electronic publications, audiovisual presentations, promotional literature, advertising, social media content, and/or other similar ways. My consent is freely given to RPHR without expecting payment. I release RPHR and their respective employees and agents from any and all liability that may arise from the use of such news media stories, promotional materials, written articles, videotapes, and/or photographs. I understand that I can revoke this release at any time in writing and that the use of any of my photos or other information authorized by this release will immediately cease.
